Matthew Winn, chair of the NHS Confederation Community Health Services Forum, on how home care and support providers combining knowledge and resources will impact on millions of people. Winn also discusses raising the profile of the community's 'vital role' in easing winter pressures & discusses challenging the NHS to 'revolutionise' the way home care and support is provided. To enable the 'cradle to grave' community support service providers merge both together and with the NHS, the 'historic and traditional boundaries' of NHS trusts need to change.
